How to become a medical cannabis caregiver in Oklahoma
Some patients choose to have – or are required to have – a designated caregiver who can purchase, transport, possess and administer cannabis products for them.
Your physician must designate the caregiver on the Physician Recommendation Form. A caregiver application may not move forward without this written designation from the physician.
OK caregivers can care for up to five licensed patients. Adult patients may only have one caregiver. Minor patients can designate up to two caregivers. Registrations expire on the same day that the patient’s registration expires.
To qualify as your caregiver, they must:
- be a family member or assistant who regularly looks after the patient
- be a resident of Oklahoma
- be designated by the patient on their own application
- possess a Physician Recommendation Form filled out and signed by the patient
- have proof of identity, and an acceptable photo
- not begin their duties until approved by the OMMA
Caregivers cannot:
- use the patient’s cannabis,
- purchase or possess more than the maximum amount allowed for the patient,
- cultivate

To qualify for a medical marijuana card in Oklahoma, you must:
