How to be a medical cannabis caregiver in Maryland

Some patients choose to have – or are required to have – a designated caregiver.

Patient caregivers can purchase and administer cannabis for a registered patient. A caregiver may serve no more than five registered patients at any given time. A patient may designate one caregiver (though minor patients may have up to four caregivers).

The process of registering is fairly straightforward. The patient and caregiver must register with the MCA. After both parties have registered, the patient will log into their account and designate the caregiver.

Caregivers must be:

  • at least 21 years old,
  • the parent or legal guardian of the patient if the patient is a minor,
  • registered with and approved by the Maryland Cannabis Administration,
  • in possession of their ID card from the Commission at all times while possessing cannabis on behalf of the patient.

Caregivers cannot:

  • use the patient’s cannabis,
  • purchase more than the maximum amount allowed for the patient,
  • serve more than five patients at a time.

The caregiver application fee is $50.

Frequently asked. Frequently answered.

  • Qualifying Maryland residents must first be diagnosed with one of the debilitating medical conditions listed above. All patients must be evaluated by a state-certified physician, physician assistant, podiatrist, dentist, nurse practitioner, or nurse-midwife and be at least 18 years of age or a minor with an adult caregiver. Qualifying patients also must have a Maryland residency or reside in a Maryland treatment center. Prison inmates can’t use cannabis, but Maryland’s law specifically prohibits a parole or probation officer from issuing a violation for the patient’s legal, authorized medical marijuana use.

  • A medical marijuana certification may be issued if the patient has a debilitating or chronic disease that causes:

    • anorexia,
    • cachexia,
    • glaucoma,
    • post-traumatic stress disorder (PTSD),
    • seizures,
    • severe or chronic pain,
    • severe nausea,
    • severe or persistent muscle spasms,
    • wasting syndrome,
    • or another chronic medical condition that is severe and for which other treatments have been ineffective.
  • This is up to your practitioner. Along with the list of conditions, a practitioner can issue a certification for “another chronic medical condition that is severe and for which other treatments have been ineffective.”

  • Maryland medical marijuana card holders can access safe, tested medicine with shorter lines and guaranteed availability. Additionally, Maryland medical patients benefit from lower taxes on purchases than recreational consumers, gain access to at-home cannabis delivery, can purchase more products, cultivate additional plants at home, and receive proper medical guidance.

  • NuggMD offers low-cost, risk-free Maryland medical cannabis evaluations, and there’s no risk because you’re only billed if you’re approved for medical cannabis. After their evaluation, medical cannabis patients are also responsible for Maryland’s $25 state registration fee.

  • Medical marijuana in Maryland isn’t reimbursable by any private, public, or VA insurance plans at this time. This may change in the future as some states are considering requiring public insurers to cover cannabis, but this is unlikely in Maryland until it is federally legalized or de-scheduled.

  • Your provider can issue your certification as soon as they have finished your evaluation. Still, it can take the state up to 30 days to review your information and approve your application (although it usually doesn’t take this long).

  • With NuggMD, you are ONLY charged for your evaluation if you are approved. We believe everyone deserves access to a qualified medical cannabis doctor, and the cost of an evaluation shouldn’t be a barrier.

  • You’ll only be charged if you’re approved for your medical marijuana certification. If not, your evaluation is free! If you sign up and pay for your evaluation, but then cancel before your evaluation, there is a small $10 fee for the payment processing. You can also change your mind after your evaluation if you wish and request a refund, but you must do so before your certification has been submitted to the state. Once you’ve been certified, a refund for services will no longer be available. Please send an email to with the subject line “Refund Request” and a brief description of your issue. We want to make sure you’re 100% satisfied with your purchase being that you, the customer, are the most important thing to us! Once a refund has been initiated, please allow 3-5 business days for the funds to transfer back to the card you used for the transaction. A refund is no longer available more than 30 days after the date of your evaluation. If any issues arise, please feel free to reach back out for further assistance.

  • Of course! You have the right to see the practitioner of your choice for your medical marijuana evaluation, so long as the practitioner is a Maryland-certified physician, physician assistant, podiatrist, dentist, nurse practitioner, or nurse-midwife who has registered with the state as a Certifying Healthcare Provider. All NuggMD practitioners are licensed in the state for which they provide evaluations and have acquired all state-required cannabis training and certifications.
