Qualifying conditions in Massachusetts
Per Massachusetts state law, new applicants and renewing patients must have one of the following debilitating medical conditions:
- Cancer
- Glaucoma
- Hepatitis C
- Amyotrophic lateral sclerosis (ALS)

The state defines “debilitating” as “causing weakness, cachexia, wasting syndrome, intractable pain, or nausea, or impairing strength or ability, and progressing to such an extent that one or more of a patient’s major life activities is substantially limited.”
How to be a medical cannabis caregiver in Massachusetts
Some patients choose to have – or are required to have – a designated caregiver. A designated caregiver can:
- transport patients to their Medical Marijuana Treatment Center (MTC)
- obtain and transport cannabis from an MTC for the patient
- cultivate for a patient that has a Hardship Cultivation Registration
- prepare and administer the cannabis to the patient
A caregiver must be at least 21 years old and possess a government-issued ID. The patient must authorize you as their caregiver by providing you with a PIN number following completion of their own application. A patient may establish up to two caregivers.
To register, you’ll need your government-issued ID and a passport-style photo. If you use a Massachusetts ID, the department may be able to use the photo in your RMV record. Once you have your PIN, ID, and photo (if using a non-MA ID), you’ll submit your application to the state’s Medical Use of Marijuana Online System.
Massachusetts caregivers may not:
- sell or divert the patient’s marijuana
- grow cannabis for a patient that doesn’t have a Hardship Cultivation Registration
- sell the cannabis they cultivate to the patient or others (it must be provided without compensation)
- consume their patient’s cannabis
- allow patients under 18 years old to possess cannabis when not in the caregiver’s presence
- serve as a caregiver for more than five patients at a time

How to register for a med card in Massachusetts

Absolutely! You have the right to see whomever you choose for your medical marijuana evaluation, so long as the practitioner is a Massachusetts-licensed physician, physician assistant, or certified nurse practitioner that has registered with the state as a Certifying Healthcare Provider.
