Register for a Kentucky Medical Card

Once the state receives your practitioner’s recommendation, you can complete your application online. Kentucky charges a $25 fee for registration. You’ll be notified within 30 days of your application status. If approved, you’ll be issued an electronic Registry ID card. Your Kentucky medical marijuana card is valid for one year.
Renew Your Medical Card in Kentucky
At the moment, the state isn’t issuing registry identification cards. Once they begin, Registry Identification Cards will be good for one year from date of issuance. Your initial evaluation must be in the physical presence of the doctor, but follow-up evaluations can be conducted via telemedicine if the physician thinks it’s appropriate.
Become a Kentucky Medical Marijuana Caregiver
Kentucky medical cannabis patients can designate a caregiver to help them purchase, store and prepare cannabis for their treatment. Patients under 18 must have a caregiver, which can be their parent or guardian.
Kentucky caregivers must obtain the following information and documents for their caregiver application:
- Name of the qualified patient
- Registry ID card number of the qualified patient
- Written certification from a registered medical cannabis practitioner for the patient
- Documentation that the patient has been diagnosed with a qualifying medical condition
- Attestation of serving as the caregiver for the identified qualified patient (included in the notarized caregiver signature form)
Caregivers will use this information to fill out their application on the state’s patient and caregiver registry portal. Then they will pay the required fee and submit it for approval.
To qualify as a caregiver, they must:
- Be 21 years or older
- Be a Kentucky resident
- Not be convicted of a disqualifying felony offense
- Sign a notarized agreement documenting their duties as a caregiver

Patients won’t receive a refund for failure to follow through with their certification process once the practitioner completes the certification requirements. It is the patient’s responsibility to complete their certification. If you’re stuck or having trouble completing your certification, please reach out to customer service and our agents will help you through the process. We want to make sure you’re 100% satisfied with your purchase being that you, the customer, are the most important thing to us! Once a refund has been initiated, please allow 3-5 business days for the funds to transfer back to the card you used for the transaction.
